I now have osteoarthritis going on in my hands, feet and ankles alongside my RA. A DEXA scan five years ago revealed my left hip has osteopenia. I have since retired from work and I exercise as much as I can to help with the anrthritis and to help with the osteopenia.

How do osteoporosis and osteoarthritis affect rheumatoid arthritis (RA)?

Osteoporosis and osteoarthritis can complicate the management and progression of rheumatoid arthritis in several ways:

1. **Increased Risk of Osteoporosis in RA**: People with RA have a higher risk of developing osteoporosis. This is due to several factors including chronic inflammation from RA, long-term use of corticosteroids for RA treatment, and a sedentary lifestyle that may be due to RA-related pain and disability. Osteoporosis makes bones brittle and weak, increasing the risk of fractures, which can further impair mobility and quality of life in individuals with RA.

2. **Joint Damage and Osteoarthritis**: While osteoarthritis is a different condition characterized by the wear and tear of joint cartilage, individuals with RA can also develop osteoarthritis as a secondary condition. The inflammation and joint damage caused by RA can accelerate the breakdown of cartilage in the joints, leading to osteoarthritis. This can exacerbate joint pain, stiffness, and loss of function.

3. **Management Challenges**: The coexistence of osteoporosis and/or osteoarthritis with RA can complicate the management of these conditions. For example, physical activity is important for managing all three conditions, but pain and disability may limit the ability to exercise. Additionally, some medications used to treat RA can have adverse effects on bone density, further complicating the management of osteoporosis.

4. **Treatment Considerations**: Treatment strategies for individuals with RA who also have osteoporosis or osteoarthritis may need to be adjusted. For osteoporosis, this might include medications to strengthen bones, along with lifestyle changes to reduce the risk of fractures. For osteoarthritis, management may focus on pain relief and maintaining joint function, possibly including physical therapy and, in some cases, surgery.

In summary, osteoporosis and osteoarthritis can significantly impact the health, mobility, and overall quality of life of individuals with RA, necessitating a comprehensive approach to management that addresses all coexisting conditions.
